Description

Fay Biccard Glick Neighborhood Center is looking for a Case Manager. Under the supervision and direction of the Director of Family Success, the Case Manager is responsible for providing direct service and support to individuals or clients. The Case Manager is responsible for handling of referrals, with coordinating participant needs, and assisting clients in developing a plan of sustainability. The Case Manager position is ideal for entry level social workers who would like to experience micro-level service work.  The Family Success Director will provide you with over 80 hours of additonal social service support, weekly social service meetings and mentoring, and oversight during your engagement with clients.  This position allows an entry level human service or social worker students to truly recieve hands on training on diverse clients.

 

OVERALL JOB DESCRIPTION:

  • Work directly with clients to assess needs and provide assistance
  • Coordinate client needs with appropriate agencies and community support groups
  • Maintain accurate data on each client in order to facilitate program services and to assess their impact on the client and their family
  • Maintains fidelity to the Wraparound process (Seven touches) when working with all assigned families.

Responsibilities

  • Assist with review of intakes and applications
  • Provide emergency assistance and basic needs to clients supported by their plan of success
  • Coordinate meetings and case management assessments utilizing thorough communication outlets
  • Coordinate clients’ needs utilizing an assessment tool
  • Develop a plan of success utilizing 7 Touches of Engagement
  • Maintain consistent follow-up and tracking to ensure documentation is available
  • Perform any other related duties as assigned by the Director of Operations

  • enrolled in Human Services or Social service degree program
